Each form includes:

• A Mortgage Pre-Approval / Commitment Letter template
• A required Settlement Agent Pre-Closing Questionnaire

The Questionnaire must be completed by your settlement professional to confirm certain transaction-specific and jurisdiction-specific information before loan documents can be generated.

Important Instructions Before Downloading

To avoid formatting or saving issues:

  1. Download and save the PDF to your computer.

  2. Open the saved file using free Adobe Acrobat Reader.

  3. Complete the Pre-Approval / Commitment Letter and your portion of the Settlement Agent Questionnaire, then save it.
